How to obtain copies of your medical records:
- The easiest way to obtain copies of your medical records is to access our portal via electronic device or computer. Accessing our portal will provide access to most medical records free of charge. If you have not been sent a link via email to access the portal or need assistance, please feel free to call our IT department at 479.495.6310.
- Another option for receiving medical records is via encrypted email. The total size must be less than 5gb or less. Image files (radiology) are too large and must be provided on a CD.
- Option 3 is for records to be burned to a CD or thumb drive. Fees for labor, supplies, and postage maybe in incurred.
- Option 4 is obtaining a copy of paper based medical records. Fees for labor, supplies, and postage maybe in incurred.
It is Chambers Memorial Hospital’s policy to complete requests in the order in which they are received.
