CASE MANAGER/DISCHARGE PLANNER
The Case Manager/Discharge Planner, in accordance with Chambers Memorial Hospital’s policies and procedures will be responsible for coordinating with the admission staff and clinical staff to facilitate the meeting of patient’s treatment needs. This position assumes responsibility for management of the discharge plan for the patients stay throughout the program. The position contacts referral sources and family members to gather clinical information for the multidisciplinary team and reports to the treatment team. The Case Manager/Discharge Planner interacts with members of the medical/clinical team to provide a flow of communication. This position accurately documents in the medical record the findings and data that supports level and intensity of service rendered. This position is a member of the multidisciplinary team and assists in facilitating the treatment team process. The Case Manager/Discharge Planner communicates with patients, families, and referral sources to ensure the positive treatment outcomes. In addition, maintains performance improvement activities within the department and participates in TQM activities. The Case Manager/Discharge Planner adheres to the highest ethical standards regarding patient care and rights and facilitates communication to outside agencies to ensure that these patient rights, care, and needs are met. Case Manager will be responsible for facilitating Coping Skills group three times a week.
Education Preferred: MSW or Higher, but will consider BSW, LPN or RN for the position.

Pay for Bachelors in-Social Work/ RN/ LPN /Case Mgr./Discharge Planner – 35,000 – 55,000 Annually
Pay for Master’s degree or Licensed Social Worker- 55,000-65,000 Annually
